This is the PerfectStorm iPhone theme, from MMi forum member ebl4287. I have to say, I’ve not found the Storm overly compelling – but then again I also never felt amped up by Vista, but could still admire the sheer scope of the Vista Perfection theme for the iPhone.
It looks to me like this theme is in that sort of league. It includes:

Safari skin
And even a keyboard skin
