I believe the relationship is as follows: you're paying iTunes to buy the game, but it's likely ngmoco's plus+ servers that monitor the amount of energy you have. When you run out of energy you can wait a day to recharge or buy more via in-app purchasing.

In-App purchases will always be tied to iTunes b/c it charges the credit card you have listed with your iTunes account. There's also the fact that Apple gets a 30% cut of all purchases, and that likely includes the in-app ones as well.

energy is used up after playing any online session. The energy level you see on your iPhone is the one that the server sees as well. Buying energy always costs the same.
