I have come to discover that many if not most times it happens because of some network interference coming from within my own phone. I have diagnosed this by using my phone and my wife and my daughter’s phones at the same time; frequently if my phone isn’t sending the signal my wife or daughter’s will. I therefore conclude that the problem lies in my own handset.

Since discovering this, I no longer screw around once Siri balks at following my command. I immediately (1) disable Siri in the settings panel; (2) reset network settings; (3) wait 10 seconds and restart the phone; (4) enable Siri. This usually takes 5 minutes and I do it every 3 or 4 days. After the network reset I do have to re-insert my passwords for my work and home wifi but that’s ok.

I know, I know, it shouldn’t happen, but it does. Now that I know how to fix it (and don’t wring my hands over it) Siri’s usefulness has greatly improved for me.
